 Referrals
to Tampa Bay Academy may be made by psychiatrists, other mental
health professionals, school staff and/or parents. The following
documents will be requested:
•Psychiatric Evaluation
•Recent Physical Exam
•Psychological Evaluation
•Recent lab tests and medication history
•Psychosocial History |
If a child is not currently
in treatment, Admissions staff will assist in obtaining the above
information. An acceptance decision will be made within 24 hours
in most cases.
