Referral
Process
The following documents may be requested:
- Psychiatric or Psychological Evaluation
- Physical within 30 days
- Dental clearance within last six months
- Psychosocial History
- Recent lab results and medication history if indicated

If a child is not currently
in treatment, Admissions staff will assist in obtaining the above
information. An acceptance decision will be made within 24 hours
in most cases.
